Having a PhilHealth ID makes it easier and faster to transact with PhilHealth especially if you need to file a claim or avail of benefits.
It is equally important as getting a Philhealth MDR or Member Data Record, although, you can file claims even with MDR alone.
Why wouldn’t you get a PhilHealth ID anyway?
It’s often overlooked by many PhilHealth members because they think it’s not a primary valid ID.
2 types of PhilHealth ID:
- Regular PhilHealth Card
- a paper ID that you can use when claiming inpatient or outpatient hospital benefits.
- free of charge
- can be used as a secondary ID or supporting document when transacting with government agencies and private institutions
- PhilHealth Insurance ID Card
- a digitized ID issued to members in the Formal Economy and Informal Sector
- ₱90 if no dependents declared
- ₱358 if you want to declare a dependent for your ID card, but you must also present at least 2 valid IDs of your dependent. (If you get this ID at the mall, the price may be a little bit higher.)
- can be used as a primary valid ID
Who can apply for a PhilHealth ID?
Employed and unemployed individuals can apply for a PhilHealth ID, but first, you need to register as a PhilHealth member.
- For employed, your HR officer will accomplish your PhilHealth membership
- For unemployed and self-employed, you need to register as a Voluntary Member at the PhilHealth office.
Once you become a member of PhilHealth, you will have your own PhilHealth Number.
Then, you may start paying your premiums or contributions to SSS so that you can avail of the benefits when the time comes that you or your dependents get sick or hospitalized.
- For employed, your contributions to PhilHealth are automatically deducted from your salary.
- For unemployed and self-employed, you may pay your contributions at the PhilHealth office or at any accredited payment center.
Important: Don’t just pay your contributions when you are about to use it.
Remember, there are certain conditions before you can avail of PhilHealth benefits like you must have paid 6 or 9 months worth of contributions in the last 12 months.
If your PhilHealth contributions are not updated, you may not be able to avail of the benefits, thus, you will have to pay the amount that is supposed to be paid by PhilHealth when you get hospitalized.
How to Apply for the Regular PhilHealth ID Card
Step 1 – Fill out the Philhealth’s member registration form or PMRF (download here)
Step 2 – Submit the PMRF to the nearest PhilHealth office with the following required documents:
- 2 pcs 1×1 latest ID picture
- 2 valid IDs
- Any other supporting documents that would be required by PhilHealth
Step 3 – This is free, no need to pay for the paper ID.
Step 4 – After your documents have been verified, you will be given your Member Data Record (MDR) and PhilHealth ID Card.
How to Apply for the New PhilHealth Insurance Card
The steps in applying for the new PhilHealth ID is almost the same as the regular ID.
Step 1 – Fill out the PhilHealth’s member registration form or PMRF (download here)
Step 2 – Submit the PMRF to the nearest PhilHealth office or kiosks with the following required documents:
- 2 pcs 1×1 latest ID picture
- 2 valid IDs
- Photocopy of official receipts of premium payments to PhilHealth or any other documents indicating the months of premium payments to PhilHealth.
- 2 valid IDs of your dependent (if you want to declare them in your ID card
- For maximum benefits, I encourage you to declare your dependents, if you have any.
Step 3 – Pay ₱90 for regular ID or ₱358 if you declare your dependents.
Step 4 – Get your new ID and check if all the information on the ID are correct.

OCT 05, 2018: I am currently at Philhealth Office in Makati, Buendia trying to get a valid digitized ID. They said do not offer the Digitized ID to Unemployed/Self-Employed Voluntary Members. Only members who are employed in companies are given the option to get the valid digitized IDs by coordinating with their company’s HR. They said this is because their outsourced suppliers of the digitized ID requested they only make them in bulks of 50-100 IDs per transaction.
Is there a way to get this valid government digitized ID?
